We are a member of Allied Physicians of Michiana, L.L.C., a managed service organization providing health care to our community. Statements will be sent from the office of Allied Physicians of Michiana and payment can be made there or in our office.

Insurance carriers have assumed an increasing role in payment of patient charges. We will file all Medicare claims and supplemental insurances if you supply us with sufficient information to do so. You will be billed for any deductible or co-payments not covered by your insurance.

We do request payment at the time of service for non-Medicare patients but will be happy to file your primary insurance for you. If your insurance requires pre-certification or a second opinion, it is your responsibility to notify us.
